The FileBound prior authorization system will be down for enhancements Friday 02/21/2025 at 2:30PM EST – Sunday 02/23/2025 at 12:00PM EST. During this time the system will be unable to accept prior authorization requests. During downtime, prior authorization requests can be submitted via email to EyeMed_UM@EyeMed.com or faxed to 513-492-6739. As always, prior authorization forms are available on InFocus. The system downtime will have no impact on EyeMed’s ability to process your requests and we will notify you of our determination within required turnaround times.  Thank you for your patience and understanding while we work to enhance your experience with the EyeMed prior authorization process. 

Please complete the required Prior Authorization form on the next page, including the required attestation and applicable clinical documentation supporting the requested procedure.

Prior Authorization Turn Around Times:

  • Medicare Advantage: Standard Requests: Medical Services 14 calendar days, Injectable Drugs 72 hours. Expedited Requests (requires letter of medical need): Medical Services 72 hours, Injectable Drugs 24 hours.
  • Medicaid & Commercial: All Requests: 3 business days from receipt of all necessary information.
  • Emergency services. Emergency services do not require precertification.
    • Notify us by completing the UM request form via provider portal on the next business day following the emergency care or by calling 844.844.0890. Attach to the form any medical records related to the emergency care.

After form submission, the EyeMed UM team will process your prior authorization request as soon as possible within the requested timeframe.  To check your case status or if you have additional questions, please call 844.844.0890.
